This programme is suitable for clients who feel able to achieve their goals independently but need help to ascertain where they need to make changes and how to set those goals for maximum success. It includes information gathering, creating a client led action plan including visualisation and goal setting. We will look at ways that blockages can occur and ways to over come them and assess any areas where other health professionals may be able to add value to their progress.
